Food Service Inspections
Aug. 15-Sept. 15

Food service inspections are made by the Henderson office of the Green River District Health Department.
A — 85 to 100 percent with no critical violations. B — Failed two consecutive regular inspections or a follow-up inspection. C — Any critical violations or 84 percent and below. A follow-up inspection is required within 10 days. No Grade — Establishment sells prepackaged foods only and does not receive a letter grade.
The Hendersonian only provides details of critical violations.

Critical violations at Rally’s:
-Presence of insects. Ants around drink machine, flies in and around the drive-through area.
-Floors not clean. Walls and ceilings not clean. Walls and ceiling coverings not easily cleanable. Our openings not protected against entry of pests.
-Nonfood-contact surfaces of equipment not cleaned at a frequency to prevent accumulation of soil residue. Inside of ice machine lacks frequent cleaning.
-Ware washing machine not properly working. Bag of detergent in hotel pan.
-Equipment is not maintained in good repair or does not allow for proper adjustment. Walk-in freezer build-up of ice in condenser unit.
-Chemical sanitizing solution containers placed on the floor.
-Lack of properly maintained hot holding unit for TCS food. Hot holding unit has one section not holding temperature.
-Food not clearly marked to indicate the date or day by which the food shall be consumed, sold or discarded. Cheese, chili in walk-in cooler past day date mark.
-Food stored uncovered. Cheese, tomatoes, onions uncovered in line cooler.
-Food in contact with soiled equipment and/or utensils. Soft drink hoses left open and in contact with ground.
-Individual disposable towels and/or hand drying device not provided at lavatory or hand sink. Paper towel dispenser in poor repair.
-Employees eating, drinking or smoking in unauthorized area. Drinks if food prep area without lids and straws.
